  • Broker / Carrier Agreement (Initial each page and Owner Operator must sign the contract).

  • Copy of Operating Authority

  • Signed and Dated W-9

  • Insurance Certificate with Inner Fire Logistics LLC as the Certificate Holder

  • *** If your authority is under SIX months old please provide THREE verifiable references of shippers you have recently hauled loads for.
